Please read carefully, if you don't follow the instructions you might not get registered to race:

Like DCD we will not be bringing forms to the meeting, please print and completely fill out EXCEPT THE ENTRY FEE SPOT, I will fill that out since the team is paying for the race.
If you are NOT a USAT member (or for some reason do not have your number on the form) please attach $10 to your form for your 1 day membership. If you pay with a check please make it out to TRICATS.
As long as you have paid dues and do not owe the team money for anything (shirts, nike order, missed races, etc) we will cover the entry fee. If you do pay what ever you need to by the 15th or your registration will not be submitted.
If your registration does not get sent in with the rest of the teams we will not reimbursing you for this race. The only way to get funding is to turn in your registration with the team.
I do not know what MCTC numbers so leave it blank unless someone knows what it is for. Fill out the rest of the College Section as it applies to you.
The forms are due MONDAY FEBURARY 15th by 6:30 PM IF YOU DO NOT GET IT TO ME BY THE TIME I LEAVE THE MEETING YOU WILL NOT BE RACING, THE OTHER OFFICER AND KEN CANNOT TAKE YOUR ENTRY FORM AFTER I LEAVE THE MEETING. If you cannot make monday meetings you have a week to make arrangements to make sure your form gets to the meeting or to me.
